I am such a pain in my insurance company's ass-- I appeal every generic "recommendation" even though it's a time intensive process, but not everyone has the time or the cranky aptitude (crank-ptitude?) to do so. "It's not hormones, it's my physical brain structure. Do you really want to risk a hospital stay so you can save money out of your formulary?" I asked once at a hearing that went all the way up. I swear, they actually had to "think" about that one for a second. I agree wholeheartedly.
